Advocacy Support Declaration of Consent

I authorise the CAHN Advocacy team to act as my Advocate. This may involve a CAHN advocacy officer attending meetings, contacting relevant professionals or personnel, speaking to, or writing to individuals and organisations with your consent and pre-approval.  This consent is given on the understanding CAHN Advocacy Team agrees:

To be independent of service providers and free from any conflicts of interest.

To provide support and assistance to enable you to get your voice heard.

This consent is also given in the knowledge that CAHN Advocacy Team cannot:

• Guarantee to be a successful outcome.
• CAHN Advocate will support me to have my views heard and listened to but cannot ensure that a particular outcome is achieved.
• Be your legal representative or take legal action but may facilitate access to sound information as your advocate

Right to Withdraw Consent

I understand that I have the right to withdraw this consent at any time.
